Financial Arrangements for Cosmetic Dentistry Services

Thank you for choosing our dental practice for your care. As our patient, we are committed to you and your treatment. We will do everything we can to ensure that you receive personalized care and that your experience with our office surpasses anything you have ever experienced with any other dental office.
Our Financial Policy is part of your treatment agreement. Below is our Financial Policy, and we ask you to read it before entering into treatment. All balances will be due at the time services are provided.

Major Service Arrangements
Mulder Family Dentistry will ask for a deposit towards the total charges for major dental procedures, or for any treatment that will take more than one hour to complete.
Insurance Company Verification and Rejected Claims
'Please note that we are not contracted providers for all insurance plans, and that your insurance policy is an agreement between you and your insurance company. However, in some cases, we will extend the courtesy of accepting assignment of benefits.
To reserve your appointment time, all co-payments and deductibles will be due at the time treatment begins. For any balance not paid by your insurance company within 45 days of your treatment, we will require either:
- An authorized credit card; or
- Prior arrangements made with our extended payment plan with CareCredit.
Not all services are covered
Depending on your insurance company’s policies, some, or perhaps all, of your dental work may not be covered. All insurance coverage needs to be verified before your dental procedure(s) begin.
In the event of an emergency, we will be happy to oblige, and will ask you to pay for the dental work at the time it is done. We will work with you to obtain reimbursement from your insurance company.
Pre-treatment estimates
We’ll notify your insurance company about all procedures we anticipate providing and your insurance company will advise us directly about payment. If you decide to begin treatment before an estimate is received, you will be responsible for the complete charges for that treatment. We can then help you to obtain reimbursement from the insurance company.
Verification of insurance does not guarantee coverage
Your insurance company might pay less than the estimated amount, and in that case, you will be responsible for the remaining amount. Regardless of the insurance company’s determination of what a usual or customary rate is, you will be responsible for payment.
Rejected Claims
If your insurance company rejects a claim because of plan exclusions, waiting periods, clauses, coverage cancellation, or any other reason, you will be responsible for the balance.
Missed Appointments
We reserve valuable time to give you high quality dental care, and we make every effort to honor all our time commitments. If you are unable to keep your appointment, and do not notify us at least 48 hours beforehand, it is our policy to charge a fee equal to the charge for a normal office visit. Please help us to serve you better by honoring your scheduled appointment times.
